Do you need a Birth Certificate or Death Record?​

There are multiple ways to obtain these documents:

By phone:  You can phone in a request at 1-800-241-8322, option 1. To use the call in feature you must have a valid major credit card. There will be a credit card surcharge fee in addition to the cost of the certificates requested. Average turn around for a phone order is four to five days plus mail time.

Online: Visit Kentucky's Vital Statistics Website to obtain an application. You may also download a printable version of the application under the "forms" tab. If you mail an application it may take up to 30 business days to receive your documents.

Walk-ins are also available at the office in Frankfort listed below, just walk in and request the certificate you need. Walk-ins must be before 3:30 EST.

Changes to Kentucky Birth Certificate must be done through the Frankfort office. Call 1-502-564-4212 to reach the state office to make changes.
