The Diabetes Education program empowers individuals with the knowledge and tools they need to prevent, manage, or live well with diabetes. Whether you're newly diagnosed, at risk, or supporting a loved one with diabetes, our program provides support and education to improve long-term health outcomes and reduce complications.
We offer evidence-based classes that cover important topics such as healthy eating, physical activity, blood sugar monitoring, medication management, and preventing complications. Our educators help participants understand how daily choices impact blood sugar and provide personalized action plans that fit real-life routines and cultural food traditions.
Through partnerships with local providers and community organizations, we also provide diabetes screenings, promote early detection, and support referrals to specialists when needed. For those with prediabetes, we offer lifestyle intervention programs designed to reduce risk and build healthy habits before diabetes develops.
Our services are open to adults and families affected by type 1 or type 2 diabetes. Whether you're seeking support for the first time or looking to fine-tune your current plan, we’re here to help you take control of your health—one step at a time.
