Diabetes Center of Excellence
What is the Diabetes Center of Excellence?
- The Diabetes Center of Excellence (DCOE) is a
new approach to improving the long-term health of people with
diabetes through care coordination, intensive patient education, and
tracking of progress toward health goals. The program will follow
the standards of care outlined by the American Diabetes
Association. The DCOE staff will provide some of the services
included in the standards of care and will also form partnerships
with existing community agencies offering diabetes services to meet
the needs of their clients.
Who is eligible for services?
- Non-pregnant adults (age 18 and older) who
have diabetes and are enrolled in Medicaid are the focus of the DCOE.
- Other non-pregnant adults with diabetes may
also receive services.
- Services will be available in Barren, Butler,
Edmonson, Hart, Logan, Metcalfe, Simpson, and Warren counties.
How does the program work?
- A patient may contact the DCOE directly to
enroll; or Medicaid or their physician may refer them.
- The Care Manager will meet face-to-face or by
phone with the patient to complete an assessment.
- Based on the assessment, the Care Manager will
work with the patient to set goals, make arrangements for needed
services, and address barriers that would make it difficult for the
patient to control his or her diabetes.
- Examples of medical services that the Care
Manager may arrange for the patient include: Primary care visits,
Medical Nutrition Therapy, eye exams, diabetes education, smoking
cessation, oral health care, and mental health care. Examples of
barriers that may be addressed include: transportation, childcare,
language, and literacy.
- DCOE staff will communicate with the patient’s
physician.
- The Care Manager will follow up with the
patient quarterly.
- The DCOE Coordinator will provide ongoing
intensive diabetes education, such as Diabetes Self-Management
Training classes and behavior change support groups, as well as
quarterly newsletters to patients.
When will the Center begin accepting patients?
- Pre-registration begins January 29, 2007.
