Perinatal Home Visiting

The perinatal home visiting program provides visits to high risk prenatal patients, high risk postpartum women and infants.  The nurse does an in-depth nursing assessment including physical, environmental and social issues, counseling regarding prenatal health, postpartum and infant care.  Outreach visits are made to local hospitals.  WIC (Women, Infant and Children) services are provided upon request in the hospital and in the home.
